Health Insurance for a Spanish Visa

No-co-payment cover that meets consulate requirements – for the Non-Lucrative, Digital Nomad and other Spanish entry visas

  • Accepted for non-lucrative, digital nomad and work visa applications
  • No co-payments, no waiting periods, repatriation included
  • Certificate issued for your consulate application – arranged from abroad

What health insurance do I need for a Spanish Non-Lucrative Visa?

For a Spanish Non-Lucrative Visa you need private health insurance from an insurer authorised in Spain, with full cover, no co-payments and, where the consulate asks, no waiting periods. Sanitas Residents Visa is built for it: it meets all four points and includes repatriation, a minimum one-year term and a consulate certificate issued when the policy activates.

Health insurance for Spain's digital nomad visa

Spain's Digital Nomad Visa requires full private cover with no co-payments and, where the consulate asks, no waiting periods, valid in all Spain for the whole stay. Sanitas Residents Visa (from €76,89 a month) is built for it. Sanitas Mas Salud (€61,40), Mas Salud Family (€62,70), Top Quantum (€64,40), Mas 90 (€83,70) and Premium 500 (€94,00) are also accepted.

Cover for non-EU citizens moving to Spain

If you are a non-EU national – from the UK, the US, Canada, Australia or anywhere outside the EU/EEA – you will normally need a visa to live in Spain, and Spanish consulates require you to hold private health insurance as part of that application. The policy has to provide full cover with no co-payments, from an insurer authorised in Spain, with no waiting periods on the cover the authorities care about. You arrange it before you travel and submit the insurance certificate with your visa paperwork. Sanitas is one of the most widely accepted insurers for these applications, and we arrange the certificate consulates ask for.

  • Sanitas Mas 90 and Sanitas Premium 500 have a maximum joining age of 64, against 75 on most of the other plans that meet Spanish visa requirements.

Which visas need this cover?

  • Non-Lucrative Visa (NLV): for those living in Spain without working – retirees and the financially independent. Full private cover with no co-payments is mandatory.
  • Digital Nomad Visa: for remote workers and freelancers employed outside Spain. Requires compliant private health insurance for the whole stay.
  • Work & highly-qualified visas: where you are not yet covered by Spanish social security, private cover bridges the gap and supports the application.
  • Golden Visa renewals: the investor scheme closed to new applicants in April 2025, but existing holders renewing their permits still include authorised private cover in their documentation.

Key facts: Sanitas Residents Visa for a Spanish visa

  • Insurer authorised in Spain: Sanitas, S.A. de Seguros is regulated in Spain by the Direccion General de Seguros y Fondos de Pensiones and is part of the Bupa group.
  • Full cover: Sanitas Residents Visa gives full and unlimited cover throughout Spain for medical, surgical and hospital care, including maternity and emergencies.
  • No co-payments and no deductible: you pay nothing at the point of service in the Sanitas network, so the premium is the only cost for covered care.
  • No waiting periods: the Sanitas Residents Visa conditions set no waiting periods, so every cover in the policy is available from the start date.
  • Repatriation included: repatriation to your home country in the event of death is part of the Sanitas Residents Visa policy, not an add-on.
  • One year, renewable: the policy runs for a minimum of one year and renews automatically each year.
  • Consulate certificate: The certificate the consulate asks for is issued at the moment of activation, so we time activation to your appointment date. It carries the policy number, the dates and each insured person named.
  • Arranged from abroad: Sanitas Residents Visa can be taken out up to 5 months before you arrive in Spain. You do not need a Spanish foreigner identity number (Número de Identidad de Extranjero) to start.
  • Digital Nomad Visa: the visa requires compliant private health insurance for the whole stay; Sanitas Residents Visa meets it with full cover throughout Spain, no co-payments, no waiting periods and repatriation included.
  • General conditions: treatment after a work or road accident is covered with the work and traffic accident add-on, and urgent care after a road accident is covered without it. The general conditions exclude alternative medicine, cosmetic treatment, fertility treatment such as in vitro fertilisation, and glasses and contact lenses. Care outside the Sanitas network is not paid, except in a life-threatening emergency.

Some consulates spell this out: the Spanish Consulate in Los Angeles asks for insurance with no deductible, no copayment, no waiting period or coverage limit. Sanitas Residents Visa is the plan built for consulates that ask for this.

HealthPlan Spain is an authorised Sanitas agent (agent code 22395) in Marbella that arranges Sanitas visa policies in English, at the same price as buying direct from Sanitas.

How to get visa-ready cover

  • 1. Get your quote from abroad. Tell us your region in Spain and dates of birth – you do not need to be in Spain yet, and you do not need an NIE to start.
  • 2. We arrange your certificate. Once the policy is set up, Sanitas provides the insurance certificate in the format the consulate expects, ready to attach to your visa file.
  • 3. Submit with your application. Include the certificate with the rest of your visa documentation at your consulate or via your gestor.
  • 4. Cover activates on your start date. Pick the policy start date that fits your move – cover and repatriation are in place from day one, with no waiting period on the visa-relevant benefits.

Frequently Asked Questions

Do I need a no-co-payment plan for a Spanish visa?

Yes. Spanish consulates require full private health cover with no co-payments for non-lucrative, digital nomad and similar visas. Plans with co-payments are generally not accepted for these applications.

Can I take out the policy before I move to Spain?

Yes, that is exactly what the Sanitas Residents Visa plan is for. You can arrange it from your home country, with no NIE required to start, and we arrange the certificate you submit with your visa application.

What certificate does the consulate need?

A certificate confirming full private medical cover with no co-payments, no waiting periods on the relevant benefits, and repatriation, from an insurer authorised in Spain. Sanitas issues this when your policy is set up and we send it to you for your application.

Is repatriation included?

Yes. The Sanitas Residents Visa plan includes repatriation as standard, which is one of the points consulates look for on visa applications.

How long does the cover need to run?

For most visas the insurance must cover the full period of the authorisation, typically a year, renewable. We set the policy up to align with your visa period.

I'm an EU citizen: is this the right page?

No. EU, EEA and Swiss citizens do not need a visa. You register as a resident instead. See our residency health insurance page.

Which Sanitas plans are accepted for a Spanish visa application?

Sanitas Residents Visa, Sanitas Top Quantum, Sanitas Mas Salud, Sanitas Mas Salud Family, Sanitas Mas 90 and Sanitas Premium 500 meet Spanish visa requirements. All six have no co-payments and are accepted for visa applications. Sanitas Residents Visa is purpose-built for applying from abroad, with no waiting periods and repatriation included.

Which Spanish visas need private health insurance?

The Non-Lucrative Visa requires full private cover with no co-payments. The Digital Nomad Visa requires compliant private health insurance for the whole stay. Work and highly-qualified visas use private cover to bridge the gap where the applicant is not yet covered by Spanish social security. Golden Visa holders renewing their permits still include authorised private cover in their documentation. Retirees from outside the EU use the Non-Lucrative Visa, because Spain has no separate retirement visa. Health insurance is also part of the application for the Family Reunification Visa, the Self-Employed Work Visa, the Internship or Training Visa and the EU Long-Term Residence Permit. For a Student Visa, Sanitas International Students is for students aged 14 to 35 who are accredited by a Spanish university or college. Sanitas Professional, the plan for the self-employed, is for people already registered as autonomo, so a Self-Employed Work Visa application uses one of the six plans on this page.

Is travel insurance accepted for a Spanish visa?

No, travel insurance is usually not enough for a Spanish visa. Spanish consulates ask for full private health insurance from an insurer authorised in Spain, with no co-payments, covering medical, surgical and hospital care throughout Spain. Cover for emergencies during a trip abroad is not the same thing.

Are there waiting periods on health insurance for a Spanish visa?

Sanitas Residents Visa has no waiting periods, so every cover in the policy is available from the start date. Sanitas Mas Salud, Mas Salud Family, Top Quantum, Mas 90 and Premium 500 carry waiting periods on some services, for example 8 months for childbirth on Sanitas Mas Salud. Where a consulate requires no waiting periods, Sanitas Residents Visa is the plan built for the application.

How much does health insurance for a Spanish visa cost?

The premium for Spanish visa health insurance depends on the age of each person and the province where you will live in Spain. The plan cards on this page show the monthly starting price for each plan, and Sanitas Residents Visa is billed every 6 months or annually. Get a quote for the exact premium for everyone applying.

Is there an age limit for health insurance for a Spanish visa?

Yes, there is a maximum age to take the policy out, but no maximum age to stay insured. The joining limit is 75 on Sanitas Residents Visa, Mas Salud, Mas Salud Family and Top Quantum, and 64 on Sanitas Mas 90 and Premium 500. Sanitas Unico has no upper age limit but cannot be used for a visa application.

Can I use a visa health insurance policy if I have a pre-existing condition?

Yes, a pre-existing condition does not stop the policy being used for a Spanish visa application. Visa applicants are underwritten like everyone else: you declare the condition on the health questionnaire, and Sanitas can accept it, exclude it from cover or decline the application. Consulates check the level of cover, not personal exclusions.

Can I arrange Spanish visa health insurance in English?

Yes, HealthPlan Spain arranges Sanitas visa health insurance in English from the first quote to the consulate certificate. We are an authorised Sanitas agent (agent code 22395) with an office in Marbella, open Monday to Friday 8am to 6pm and Saturday 9am to 2pm by phone or email. You pay the same premium as buying direct from Sanitas.

Do US citizens need health insurance in Spain for a Non-Lucrative Visa?

Yes. US citizens applying for a Non-Lucrative Visa or Digital Nomad Visa must show private health insurance from an insurer authorised in Spain. It needs no co-payments and, where the consulate asks, no waiting periods. Sanitas Residents Visa meets those points and includes repatriation. You can set it up from the United States, and we arrange the consulate certificate.

What happens if my visa is refused?

Tell us as soon as you have the consulate decision. We can usually cancel the policy and issue a refund of the unused portion of the premium, subject to Sanitas terms. Send the refusal letter and we handle the cancellation.

How quickly do I get the quote and the certificate?

Tell us the ages of the applicants and the region in Spain and we confirm the figure in writing the same day. The certificate the consulate asks for is issued at the moment of activation, so we time activation to your appointment date.

Do I pay the doctor first and claim the money back?

No. You show your card at Sanitas network centres, with nothing to pay at reception. You pay the provider directly and then claim back the eligible percentage only on the reimbursement route, where a plan has it: Sanitas Mas 90 pays 70% of eligible costs at providers outside the network, and Sanitas Premium 500 reimburses 90%.
