Sanitas Health Insurance for the Self-Employed

Tax-deductible cover built for autonomos – and your spouse and children up to 26

  • Up to €500 / year per insured deductible against your IRPF
  • Sick-pay support: up to 3 monthly premiums refunded if you can't work
  • Work and road accident cover included – often excluded by other insurers

Why do self-employed autonomos in Spain take out private health insurance?

Registering as autonomo in Spain's special social security regime for the self-employed (RETA) gives you public healthcare, but a wait for a scan or an operation can cost you income. Sanitas Professional (no co-payments, from 81,00 € a month) and Sanitas Professional Plus (from 51,40 €) add private specialists without public waiting lists and Total Protection sick-pay support.

Health insurance designed for the self-employed in Spain

The Sanitas Professional range is built specifically for autonomos - freelancers, sole traders, consultants, contractors and self-employed business owners. Unlike standard policies, both plans on this page include sick-pay support and work and road accident cover, recognising that when you are self-employed, an illness or injury directly stops your income. Sanitas Professional adds funeral cover after a fatal accident, up to €5,250 per person. You also benefit from one of the most powerful features Spanish tax law allows the self-employed: private health insurance premiums for you, your spouse and children under 25 who live with you are deductible against your IRPF, up to €500 per insured per year. Cover is available from age 0 to 75 with no maximum age to remain insured, and you can extend the policy to your spouse and children up to age 26.

The autonomo tax break: up to €500 / year per insured

Spanish tax law (Art. 30.2.5 of the IRPF Act) lets self-employed workers deduct private health insurance premiums for themselves, their spouse and their children under 25 living with them, up to a limit of €500 per insured per year - or €1,500 per insured per year where any of those persons has a recognised disability. In practice this means a typical autonomo with a spouse and two children can deduct up to €2,000 per year against their professional income, recovering a meaningful percentage of the premium back through their annual IRPF declaration.

  • Who qualifies: any worker registered as autonomo with Hacienda and the RETA (Regimen Especial de Trabajadores Autonomos), determining their income under estimacion directa (normal or simplified). The deduction also applies to the autonomo's spouse and children under 25 who live with them.
  • Limits: €500/year per insured, raised to €1,500/year per insured with a recognised disability. The deduction reduces the net income on which your IRPF is calculated - it is not a tax credit, but a deductible business expense.
  • What you need: the policy in your name as the autonomo, and premiums paid by you. A personal account or a business account both count. Sanitas issues a year-end certificado de primas pagadas. We send this certificate automatically to every Professional / Professional Plus customer in January.
  • Worth it? For most autonomos the deduction recovers the equivalent of several months of premium per year. Combined with the sick-pay protection on top, the Professional range is the most cost-efficient private cover for self-employed workers in Spain.

*Tax information is general and based on Spanish IRPF rules in force at the time of writing. We are health insurance specialists, not tax advisors - speak to your gestor or asesor fiscal to confirm how the deduction applies to your specific situation.

  • How often do you actually use private care? If you visit a GP, specialist or run regular tests every month, the no-copayment Professional plan is almost always cheaper across the year. If you mostly use the policy for emergencies, surgery and the occasional specialist, the Plus version costs less per month and the €360 annual cap protects you from surprises.
  • Are you covering a family? Both plans extend to your spouse and children up to 26. For a family of 3 or more, the no-copayment plan removes the friction of every paediatric visit, vaccination check or specialist appointment generating a fee.
  • What about the sick-pay protection? Both plans include the Proteccion Total benefits package - up to 3 months of premium refunded, home help, taxis to keep you working, prescription delivery, physiotherapy at home, taxi/childcare for kids when you can't move. This is the differentiator most autonomos pick the range for.
  • Add-ons: both plans accept the standard Sanitas add-ons - expanded dental, gynaecology/paediatrics reimbursement, pharmacy reimbursement, and reimbursement of medical costs up to €300,000. Talk to us when quoting and we'll size them against your situation.

Frequently Asked Questions

Who can take out a Sanitas Professional policy?

The Professional range is designed for workers registered as autonomo in Spain - freelancers, sole traders, contractors, consultants and self-employed business owners. The plan also covers the autonomo's spouse and children up to 26 years old.

Is the premium tax deductible?

Yes. Under Article 30.2.5 of the IRPF Act, an autonomo paying premiums for themselves, their spouse or their children under 25 living with them can deduct the premiums as a business expense, up to €500 per insured per year (€1,500 per insured per year where the insured has a recognised disability). Sanitas issues an annual certificado de primas pagadas to support the deduction.

What is the difference between Professional and Professional Plus?

Professional has no co-payments at all - you pay your monthly premium and that is it. Professional Plus is cheaper per month but adds small co-payments per service used (€0 for the first six services, then €4, €7 and €10 as you use more), capped at €360 per year per insured. High-frequency services such as physiotherapy and respiratory therapy are €12. Both versions include the same sick-pay, work-accident and family cover.

What does the sick-pay (Proteccion Total) actually pay?

If you are immobilised at home or in hospital due to Temporary Disability or hospitalisation for more than 72 hours, Sanitas refunds the corresponding monthly premium. The benefit is capped at 3 monthly premiums or €200 a year per policy. There is no waiting period when the cover starts. Between two claims, Sanitas applies no wait after an accident, 1 month for a different illness and 6 months for the same illness. The benefit is valid up to age 75. Total Protection does not pay out for maternity leave, cosmetic treatment, back pain without scan evidence, headaches, depression or stress.

Are work and road accidents really covered?

Yes. Healthcare for an accident at work, a road accident or a professional illness is covered on both the Professional and Professional Plus plans - including access to the full Sanitas medical chart, with no extra payments and no paperwork. This is one of the most important differences against generic health policies, which usually exclude these accidents.

What does the accidental death cover provide?

On Sanitas Professional only, if an insured person dies in an accident, Sanitas arranges the funeral service and transfer within Spain through providers it appoints, up to €5,250 per person. It is not paid as cash on top of the funeral. The heirs receive the €5,250 instead only if Sanitas could not provide the service for reasons beyond its control, or the funeral was carried out through other means. Sanitas Professional Plus does not include this cover. To activate the cover the family calls us on (+34) 952 835 600 straight away, and we arrange it with Sanitas, which needs the official medical death certificate.

Are there waiting periods?

GPs, specialists and routine care start from day one. The standard Sanitas waits apply to high-cost services: 3 months for OMC group 0-II surgical procedures, 6 months for complex diagnostic tests and psychology, 8 months for childbirth or caesarean, and 10 months for hospitalisation, OMC group III-VIII surgery, complex therapeutic methods and tubal ligation/vasectomy.

Can I cover my spouse and children on the same policy?

Yes. Both plans accept the autonomo, their spouse and their children up to 26 years old on the same policy - one renewal date, one tax certificate, and a single point of contact for the whole family.

What are the maximum contracting and renewal ages?

You can take out the policy from age 0 to 75. There is no maximum age to remain insured - once on the policy you can keep renewing for life.

Am I covered when I travel outside Spain?

Both plans include emergency medical cover abroad for trips of up to 90 days, with an annual limit of €12,000 per insured. For frequent international travellers we usually recommend pairing the policy with a worldwide reimbursement add-on or moving to one of our international plans.

Who qualifies for the autonomo health insurance tax deduction?

The autonomo tax deduction applies to any worker registered as autonomo with Hacienda and the RETA (Regimen Especial de Trabajadores Autonomos) who determines income under estimacion directa. The deduction also applies to the autonomo's spouse and children under 25 who live with them. The policy must be in the autonomo's name and the premiums paid by the autonomo, from any account in their name. Sanitas issues a year-end certificado de primas pagadas, sent to customers each January.

What home support does Sanitas Proteccion Total provide?

Sanitas Proteccion Total supplies a home assistant for housework and childcare, taxis to keep the autonomo working through the first two weeks, prescription delivery and physiotherapy at home. The support applies when the policyholder is immobilised at home or in hospital for more than 72 hours due to Temporary Disability. Cover for the policyholder runs up to age 75.

Do autonomos get public healthcare in Spain?

Yes, an autonomo registered in Spain's special social security regime for the self-employed (RETA) gets public healthcare on the same footing as an employee, with dependants covered too. The public system treats you in its own time, and for an autonomo a wait for a scan or an operation can mean lost income. Private cover such as Sanitas Professional adds specialists and surgery without the public waiting lists.

How much does health insurance for the self-employed cost in Spain?

Sanitas Professional, with no co-payments, costs from 81,00 € a month, and Sanitas Professional Plus, with co-payments capped at €360 a year per insured, from 51,40 € a month. Your price depends on your age and where you live in Spain. An autonomo taxed under direct assessment can deduct up to €500 a year per insured from taxable income.

How do I apply for a Sanitas self-employed plan in English?

Compare your price for both plans on our self-employed plans comparison, then apply through HealthPlan Spain in your own name, in English. HealthPlan Spain is an authorised Sanitas agent (agent code 22395) in Marbella, and you pay the same premium as buying from Sanitas.
