Is Dental Insurance Worth It in Spain?
Yes, for most people who see a dentist at least once a year. Sanitas Dental Milenium and Dental Premium include routine services at no charge and the rest at fixed discounted prices, with no waiting periods, no upper age limit and children under 6 free. Added to a Sanitas health policy, Dental Milenium costs €11,20 a month.
Who arranges it: HealthPlan Spain is an authorised Sanitas agent (agent code 22395) in Marbella that arranges Sanitas policies in English, at the same price as buying direct from Sanitas.
Dental care in Spain is almost entirely private. The public health system (Seguridad Social) covers little more than emergencies, extractions and basic care for children, so for check-ups, cleanings, fillings, crowns and orthodontics most people pay a private dentist directly. That is where a dental plan earns its keep - and this guide walks through what treatment actually costs, how Sanitas dental cover works, and how to judge whether a plan is worth it for you.
What does dental treatment cost in Spain?
Private prices vary by clinic, city and the dentist's own list, but these are the typical ranges you can expect to pay out of pocket without cover:
- Check-up or consultation: €30 - €50
- Scale and polish (hygiene): €50 - €80
- X-ray: €15 - €40
- White filling: €50 - €100
- Extraction: €50 - €120
- Root canal (endodontics): €150 - €350
- Crown: €400 - €700
- Dental implant: €800 - €1,800
- Orthodontics (braces or aligners): €2,000 - €5,000
A single routine year - one check-up, a hygiene appointment and a couple of x-rays - can easily reach €100 to €170 before you have had any actual treatment. Add one filling or a crown and the bills climb quickly.
How Sanitas dental insurance works
Sanitas dental plans work in two parts. A list of routine services is included in the premium at no extra charge - typically consultations, cleanings, x-rays and basic procedures. Everything outside that list is charged at a set, pre-agreed price you pay directly to the dentist, with a Sanitas dental discount of up to 40% already applied. The 2026 rate card lists the exact price of every procedure before you go, so there are no surprises at the chair.
The key things to know:
- No waiting periods and no pre-existing exclusions - cover starts on your policy start date and you can use it from day one.
- Children under 6 are included free on a parent's policy; the premium only begins on their sixth birthday.
- 210+ Milenium dental clinics across Spain: see the Milenium dental clinics on our map. Dental Premium adds a wider network of more than 2,000 dental professionals, and Sanitas Dental over 1,000.
- A co-payment only where it applies - on the with co-payment plans you pay just €3 per day of visit, however many services you have that day; the no co-payment versions have none. (Health-plan co-payments work differently – see how those work.)
What you pay depends on whether you buy dental on its own or add it to a Sanitas health policy, and adding it is always cheaper. Added to a health policy, Dental Milenium costs €11,20 a month; Dental Premium and Sanitas Dental cost more, and each plan is priced on our dental plans page. Bought on their own, Milenium and Premium each come with or without co-payments, and Sanitas Dental comes in one version with a €3 co-payment per service used. If you already hold a Sanitas Mas Salud or Professional health plan, Sanitas Dental 21 is included free, more than 40 services plus 21% off everything else, and you can top up to a higher dental plan if you want more.
So is dental insurance worth it?
The honest answer is that it depends on how much dental work you and your family have - but the maths tips in favour of a plan more often than people expect.
Added to a health policy, the entry plan costs less each month than a single private check-up. In a quiet year with only routine care, the check-ups, hygiene visits and x-rays that would otherwise cost you €100 to €170 are largely included, so the plan roughly pays for itself on prevention alone. Add a single filling and you are already ahead.
In a year with bigger treatment the case is clearer still. With up to 40% off a crown or an implant, one piece of significant work can save more than several years of premiums. Because there are no waiting periods, you are covered from the moment the policy starts, so you do not have to plan a year ahead.
For families it is often a straightforward yes: children under 6 cost nothing to add, and children tend to need regular check-ups, sealants and the occasional filling as their teeth come through.
Where a plan is least likely to pay off is if you genuinely never visit the dentist. But since prevention is what keeps the expensive work away, that is a gamble on your own teeth.
