How Much Is Health Insurance in Spain?

How Much Is Health Insurance in Spain?

On the 2026 Sanitas tariff, a healthy adult aged 30 to 39 living in mainland Spain pays between €29.00 and €116.82 a month for private health insurance. The price depends on the province and on whether the plan has copayments. Those are real figures from our own price list, quoted the way our calculator quotes them for one person, not estimates. The table below gives every age band.

Who arranges it: HealthPlan Spain is an authorised Sanitas agent (agent code 22395) in Marbella that arranges Sanitas policies in English, at the same price as buying direct from Sanitas.

Like most types of insurance, Sanitas health cover doesn't cost the same for everyone. The price you pay depends on factors such as your age, your region and the type of cover you choose, and whether or not you include copayments or extras like dental or pharmacy coverage.

Copayments or no copayments: what it changes

At 30 to 39 the gap is €29.00 to €32.71 a month with copayments against €89.53 to €116.82 without. A copayment plan keeps the premium low and charges a fixed amount each time you use it. In between sits Sanitas Mas Salud Optima at €58.97 to €79.81 for the same age band, with wider cover than Accede. Its copayment starts at €4 a visit for the first three services in a year and steps up to €15 from the sixteenth, with separate fixed amounts for emergencies and complex tests. If you expect to use the policy often, or you need it for a visa or residency application, the no-copayment plan is the one to price. How copayments work explains the mechanics.

What Affects the Cost of a Sanitas Policy?

  • Age: Premiums rise with age, so younger applicants pay less than older ones.
  • Region: Your province of residence affects pricing due to differences in local medical costs.
  • Number of people covered: Individual, couple, and family plans are priced accordingly. On the Sanitas Mas Salud range, a couple avoids the single-person loading, and discounts start at three or four people insured.
  • Plan type: Entry-level policies like Sanitas Accede cost less than comprehensive options like Sanitas Mas Salud Plus or Sanitas Premium 500.
  • With or without copayments: Plans with co-payments are cheaper, while those without offer full coverage for a higher premium.
  • Extras: Adding dental, optical, or pharmacy coverage increases the cost.

Do foreigners pay more for private health insurance in Spain?

No. Sanitas prices a policy by age, province, plan and the number of people insured, not by nationality. Citizens of the United Kingdom (UK), the United States (US) and the European Union (EU) read their price from the table above, the same as anyone else of their age in their province.

What can change is the plan you need. Most visa applicants from outside the EU need full cover with no copayments, such as Sanitas Mas Salud, so the copayment plans in the table do not qualify. EU citizens registering as residents who are not working or paying Spanish social security need the same. Non-EU nationals applying for a Spanish residence visa from outside Spain can also choose Sanitas Residents Visa, which has no copayments and no waiting periods. Our pages on health insurance for a Spanish visa and health insurance for residency set out the rules.

Example: What You Might Pay

A 35-year-old living in Valencia, insuring only themselves, pays roughly this on the 2026 tariff. These are real figures from our own price list, not estimates:

  • €29 a month for a basic plan where you pay a small amount each visit (Sanitas Accede)
  • €67.53 a month for a fuller plan where you still pay a small amount each visit (Sanitas Mas Salud Optima)
  • €96.04 a month for full cover where you pay nothing at the clinic, including hospital stays, maternity and mental health support

Prices vary based on your personal quote and optional extras.

How Can You Lower Your Premium?

  • Choose a plan with copayments
  • Pay annually instead of monthly: Sanitas offers a 4% discount for annual payment on every plan in the table except Sanitas Unico, which already carries its standard discount whichever way you pay
  • Only cover the services you need (e.g. outpatient only)

Do Premiums Go Up Each Year?

Yes, for three reasons. Your premium at renewal depends on:

  • Your age band: you are priced at the age you have reached, so crossing into a new band moves you up a step
  • The tariff Sanitas has in force on your renewal date, which can differ from the new-member list
  • The end of any first-year discount, which ends or steps down at the first renewal
  • Our guide to changing your Sanitas plan at renewal explains each one and what you can do about it

Is health insurance cheaper in Spain than in the United States?

If you are comparing Spain with the United States, here are the numbers, each with its source. In the United States the average employer plan cost $9,325 a year for single coverage in 2025, about $777 a month, of which the worker paid $1,440. The average deductible was $1,886 before most cover begins (KFF Employer Health Benefits Survey, 2025). Bought without a subsidy on the Affordable Care Act (ACA) marketplace, the 2026 benchmark silver plan for a 40 year old averages $625 a month. The average marketplace deductible for 2026 is $3,786 (KFF).

In Spain, the table above puts one adult aged 40 to 49 at €31.30 to €35.31 a month on Sanitas Accede with copayments, or €93.96 to €133.94 for full cover on Sanitas Mas Salud. There is no deductible on either: the premium is the whole cost, apart from the fixed €4 to €15 copayments on the plans that have them.

Spanish figures are one adult insured alone, mainland provinces, 2026 Sanitas tariff, from the table above. US figures: KFF Employer Health Benefits Survey 2025 (premiums and deductibles, single coverage), KFF marketplace benchmark premiums for a 40 year old, plan year 2026. Health outcomes and spending: OECD Health at a Glance 2025 country notes (spending in US dollars at purchasing power parity, life expectancy at birth). Currencies are shown as published, not converted.

Two caveats. Spanish private insurance is medically underwritten, so pre-existing conditions are assessed when you apply and may be excluded, whereas ACA plans in the United States cannot refuse or surcharge them. And private cover here sits alongside a public system that most new arrivals cannot use straight away, which is why the visa and residency rules ask for a no-copayment plan such as Sanitas Mas Salud.

Frequently asked questions

How much is health insurance in Spain per month?

At age 30 to 39 on the 2026 Sanitas tariff, one adult insured alone in mainland Spain pays from €29.00 a month for Sanitas Accede, which has copayments. The top of the range is €116.82 for Sanitas Mas Salud with no copayments. Prices rise with age: at 60 to 69 the range is €48.10 to €259.53. The table above gives every band.

Does a couple pay per person for health insurance in Spain?

Yes. Each partner is priced at their own age, and the two premiums together are what you pay a month. On Sanitas Mas Salud and the other plans in that family, Sanitas adds an 8% loading when only one person is insured. Two adults on one policy therefore pay about 7% less each than a single adult would. Sanitas Accede and Sanitas Unico charge the same per person whether one or two of you are insured. Our guide to couples health insurance in Spain explains the rules for two people on one policy. Get a quote to see the exact couple price for your ages and province.

How much is health insurance in Spain for international students?

Sanitas International Students, for students aged 14 to 35, costs between €48.25 and €72.34 a month on the mainland for an adult student, has no copayments and meets the Spanish student visa requirement.

Do you need private health insurance for a Spanish visa or residency?

Yes for most non-EU visa applicants, and for EU citizens registering as residents who are not working or paying Spanish social security. Consulates require full cover with no copayments for a visa, and the residency registration asks for the same, so the copayment plans in the table, including Sanitas Accede, do not qualify. Sanitas Mas Salud does.

Is private health insurance in Spain more expensive for foreigners?

No. Sanitas prices a policy by age, province, plan and the number of people insured, not by nationality, so UK, US and EU citizens pay the prices in the table above. What can change is the plan you need: most non-EU visa applicants need full cover with no copayments, such as Sanitas Mas Salud.

How much is private health insurance in Spain with a pre-existing condition?

Usually the same as without one. Sanitas prefers to exclude a declared condition rather than charge extra for it, so most people with a declared condition pay the standard price for their age and province. A loading on the premium is possible but rare. Our guide to pre-existing conditions explains how Sanitas decides.

How much does it cost to see a doctor in Spain with insurance?

On Sanitas Mas Salud, nothing: there are no copayments, so consultations, tests and hospital stays are covered by the premium. On a copayment plan you pay a fixed amount per visit. On Sanitas Mas Salud Optima that starts at €4 for the first three services in a year and steps up to €15 from the sixteenth.

Do UK citizens need private health insurance in Spain?

Unless you qualify for the public system, yes. UK state pensioners with an S1 form, and people working and paying Spanish social security, can usually register with the public system. Everyone else, including most visa applicants and early retirees, needs private cover, which is why the prices above matter.

Is healthcare better in Spain or the USA?

On the outcome measures the OECD publishes, Spain scores higher: life expectancy is 84 years against 78.4, and preventable mortality is 92 per 100,000 people against 217. Spain spends about a third as much per person ($5,346 against $14,885, Health at a Glance 2025). The United States has more scanners per head and higher satisfaction with the availability of care. For a new resident the practical difference is price: full private cover for one adult in their forties costs €93.96 to €133.94 a month here, with no deductible, against a 2026 ACA benchmark of $625.
