Does Health Insurance Cover Therapy and Mental Health in Spain? What Sanitas Pays For

Does Health Insurance Cover Therapy and Mental Health in Spain? What Sanitas Pays For

Yes, private health insurance in Spain covers mental health, within limits. Every Sanitas plan on this page includes psychology, 15 sessions a year on most plans and 20 on Sanitas Mas Salud Family and Sanitas Top Quantum. Psychiatry is covered as a specialist consultation from day one, and psychiatric admission is included on the plans with hospital cover. The catch most people miss is the six-month wait for psychology on a new Mas Salud, Optima or Plus policy. Privately, a psychologist in Spain charges about €50 to €90 a session, so 15 covered sessions are worth roughly €750 to €1,350 a year.

This guide sets out the sessions, the waiting periods and the hospital cover plan by plan, and what is excluded. It then covers what therapy costs without insurance, how a psychologist is actually prescribed, and what to do in a crisis.

Sessions are per insured person per policy year. On every plan except Mas Salud Family they are on prescription and with prior authorisation. Emergencies are covered from day one on every plan. Psychiatric admissions cover acute episodes only. On every plan with hospital cover, the single room with a companion bed that comes with other admissions does not apply to psychiatric admissions. The waiting periods are listed on each plan page, or compare the Sanitas plans side by side.

What mental health cover does not include

The exclusions are the same on every plan. Psychology covers individual sessions to treat a disorder that responds to psychological intervention. Excluded are psychoanalysis and psychoanalytical therapy, hypnosis, narcolepsy treatment, animal-assisted therapy, and psychosocial and neuropsychiatric rehabilitation services. Psychometric tests are paid for by the member. Psychiatric admission covers the treatment of acute episodes only, capped at 50 days a year on the Mas Salud plans and 60 on Top Quantum. A long stay or a maintenance admission for a chronic condition is therefore not covered, while outpatient psychiatry consultations carry no such limit. Anything you have already been treated for is assessed at underwriting, which is covered below.

What therapy costs in Spain, with and without insurance

Paying privately, a standard 50-minute individual session with a psychologist in Spain costs about €50 to €90 in 2026, and online sessions a little less. Couples and family sessions cost more, and experienced trauma or eye movement desensitisation and reprocessing (EMDR) specialists charge €100 or more. A course of therapy runs quickly into four figures.

With Sanitas the covered sessions cost nothing on a no-copayment plan such as Sanitas Mas Salud. On a copayment plan the plan's copayment schedule applies to each consultation. Either way the cap is a number of sessions, not a euro amount, which is what mental health insurance is worth against private rates.

The six-month waiting period for psychology

Psychology is not covered for the first six months on Sanitas Mas Salud, Optima and Plus, and it is the waiting period people are least likely to expect. Sanitas Mas Salud Family has no wait at all, Top Quantum is three months, and Accede, Avanza, Unico and Sanitas Residents Visa have no waiting period for psychology. If you have held private cover in Spain with another insurer for at least a year, Sanitas can waive the six months when you switch. The full list of Sanitas waiting periods is in its own guide.

Psychiatry is different. It is a medical specialty and the consultation is covered from the day your policy starts. If you need to be seen quickly on a new policy, a psychiatrist is available to you when a psychologist is not. Every plan also includes the Blua digital services, which carry the Take Care of Your Mind online psychology service and a psychology coaching programme in the app from day one. That is a digital service, not a prescribed psychology session.

Psychology versus psychiatry cover: they are not the same thing

Worth knowing before you book, because people routinely ask for the wrong one.

A psychologist provides talking therapy: sessions where you work through anxiety, depression, grief, trauma or stress. They cannot prescribe medication. This is what most people are looking for, and it is the one with the waiting period, the session cap and the prescription requirement.

A psychiatrist is a medical doctor. They diagnose, they prescribe, and they manage conditions where medication is part of the treatment. Covered from day one, with no waiting period and no session cap, and you can book one directly like any other specialist.

Many people end up seeing both. The Sanitas network has more than 900 clinics with a psychologist across every province and more than 350 with a psychiatrist. Browse psychologists and psychiatrists by province in our directory.

Pre-existing mental health conditions

Anything you have been treated for before the policy starts, including medication for anxiety or depression, is assessed when you apply, the same as any other declared condition. The outcome is acceptance, acceptance with that condition excluded, or, if the risk is too high, a declined application; a premium loading is possible but rare, and Sanitas prefers an exclusion to a higher premium. Declare it. A claim for an undeclared condition can be refused and the policy cancelled. Sanitas Unico is the exception: it has no health questionnaire and no pre-existing condition exclusions. Our guide to pre-existing conditions explains the assessment step by step.

How to get a psychologist through Sanitas

Unlike other specialists, a psychologist cannot be booked directly on the policy. Sanitas requires the sessions to be prescribed, and on every plan except Sanitas Mas Salud Family the route is the same. On Mas Salud Family the 20 sessions need no prescription or authorisation, though its eating-disorder sessions do, from a psychiatrist, with a psychiatric report. Elsewhere the route is:

  1. See a Sanitas GP, psychiatrist, paediatrician or oncologist, in person or by video or telephone through the app. They prescribe the psychology sessions.
  2. Request authorisation for the sessions in the app.
  3. Book a psychologist from the Sanitas network and check your plan's session allowance before you start a course of therapy rather than halfway through it.

If you are choosing a plan with mental health in mind: Sanitas Mas Salud Family has the most sessions and no wait. Sanitas Mas Salud covers everything else in full. Sanitas Accede gives 15 sessions from day one at the lowest price but has no hospital cover. Is mental health care free in Spain?

Partly. Residents entitled to the public system can see a psychiatrist through a GP referral, and some psychology is available. Access to talking therapy is limited and waits are long, which is the gap private cover fills. Two services are free to everyone in Spain, resident or not. The first is 024, the national helpline for anyone with suicidal thoughts or worried about someone else. It is a Ministry of Health service, free, confidential and open 24 hours a day by phone or chat. The second is 112 for any emergency. The public system also provides emergency care to everyone. Frequently asked questions

Does private health insurance cover therapy in Spain?

Yes, within limits. Sanitas plans include psychology, 15 sessions a year per person on most plans and 20 on Sanitas Mas Salud Family and Sanitas Top Quantum. The sessions are on prescription from a Sanitas GP, psychiatrist, paediatrician or oncologist and with prior authorisation; Mas Salud Family is the exception and needs neither. Psychiatry is a specialist consultation and is covered like any other. Sanitas Mas Salud, Optima and Plus apply a six-month waiting period to psychology; Mas Salud Family, Accede, Avanza, Unico and Residents Visa have none.

Is therapy covered by Sanitas?

Yes. Every plan on this page includes psychology, capped at 15 sessions a year on Mas Salud, Optima, Plus, Accede, Avanza and Unico and 20 on Mas Salud Family and Top Quantum. Residents Visa includes it with cover limits. On every plan except Mas Salud Family a Sanitas GP, psychiatrist, paediatrician or oncologist prescribes it and you request authorisation in the app, then you book a psychologist from the network.

How much does therapy cost in Spain?

Privately, a standard 50-minute session with a psychologist in Spain costs about €50 to €90 in 2026, with online sessions a little cheaper and experienced trauma specialists charging €100 or more. How much is a 50-minute therapy session in Spain?

About €50 to €90 privately in 2026, or nothing for the covered sessions on a no-copayment Sanitas plan such as Mas Salud. Couples and family sessions are priced higher, group sessions lower.

Is mental health care free in Spain?

Partly. The public system covers psychiatry through a GP referral and some psychology, but access to talking therapy is limited and waits are long, which is why private cover is bought. Two things are free to everyone in Spain: the 024 helpline for anyone with suicidal thoughts, 24 hours a day by phone or chat, and 112 for an emergency.

How many psychology sessions does Sanitas cover?

15 per insured person per policy year on Sanitas Mas Salud, Optima, Plus, Accede, Avanza and Unico, and 20 on Sanitas Top Quantum. Sanitas Mas Salud Family covers 20 with no prescription or authorisation needed, rising to 40 a year for eating disorders, which do need a psychiatrist's prescription and a psychiatric report.

Is there a waiting period for psychology with Sanitas?

Six months on Sanitas Mas Salud, Optima and Plus, three months on Top Quantum, and none on Mas Salud Family, Accede, Avanza, Unico or Sanitas Residents Visa. If you have held private cover in Spain for at least a year, Sanitas can waive the wait when you transfer. Psychiatry consultations have no waiting period on any plan.

Does Sanitas cover pre-existing mental health conditions?

They are assessed at underwriting like any other declared condition: accepted, accepted with that condition excluded, or, if the risk is too high, declined; a premium loading is possible but rare. Declare anything you have been treated for, including medication, when you apply. Sanitas Unico is the exception, with no health questionnaire and no pre-existing condition exclusions. See our guide to pre-existing conditions for how the assessment works.

Does Sanitas cover psychiatric hospitalisation?

Yes on the plans that include hospitalisation, which is every plan here except Accede, Avanza and Unico. An admission covers the treatment of acute episodes only and is limited to 50 days per insured person per year on the Mas Salud plans and 60 on Top Quantum. Non-emergency admissions wait 10 months on Mas Salud, Optima, Plus and Family and 8 months on Top Quantum for a new member; Residents Visa has no wait, and emergencies are covered from day one.

What does private health insurance cost in Spain?

On the 2026 Sanitas tariff a healthy adult aged 30 to 39 living in mainland Spain pays between €29.00 and €116.82 a month, depending on the province and on whether the plan has copayments. The full price-by-age table is in our cost guide.
