Can you still flash your EHIC or GHIC card at the clinic and get free healthcare in Spain? The short answer in 2025: yes — but only in specific situations, and not without caveats. Here’s what’s changed and what every UK tourist or short-stay visitor needs to know before boarding a flight.

EHIC vs GHIC: What’s the Difference?

The European Health Insurance Card (EHIC) and its replacement, the Global Health Insurance Card (GHIC), give UK citizens access to state-provided healthcare during temporary stays in EU countries, including Spain. But they’re not the same thing — and in 2025, the original EHIC is quickly becoming obsolete for most Brits.

If your old EHIC is still valid (they expire up to 5 years from issue), you can use it in Spain. Once it expires, you must apply for a GHIC instead — and GHICs are not accepted in every EU nation the same way. Crucially, GHICs do not cover non-EU countries like Switzerland, Norway or Iceland, even though EHICs did.

How to Check if Your EHIC or GHIC Is Still Valid

If your EHIC was issued before 2021, check the expiry date — many have now lapsed. GHICs are valid for 5 years. You can apply for or renew a GHIC for free via the official NHS website.

Beware scams: there is no cost to apply for a GHIC. Third-party websites offering to process applications for a fee are unofficial.

How It Works for UK Expats in Spain

If you live in Spain permanently, neither EHIC nor GHIC is meant for you. Long-term residents must register with the Spanish healthcare system or have private cover. Even if you're awaiting residency documents, relying on a GHIC could lead to denied treatment or bureaucratic trouble down the line.

If you’re a pensioner or S1 holder, you may be issued a special EHIC through the NHS Business Services Authority — but this is separate from standard GHICs and requires official registration.

What If You Forget Your GHIC or EHIC in Spain?

You may be asked to pay upfront and then seek reimbursement later — which can be slow and complex. Always carry your card and ensure it's valid. If you're waiting for a new card to arrive, request a Provisional Replacement Certificate (PRC) to prove your entitlement.
