How Much Does an MRI Scan Cost in Spain?
A private MRI scan in Spain costs about €85 to €375 at published 2026 prices; Operarme's Madrid list price for one body area, results included, is €229. With Sanitas the same scan costs €0 on Sanitas Mas Salud, a €12 copayment on Optima and Avanza, or one service on the Mas Salud Plus scale (€0 for the first six services of the year, then €4, €7 or €10), once your doctor has requested it and Sanitas has authorised it. New members wait six months for MRI unless they transfer after a year with another Spanish insurer; emergencies are covered from day one.
This guide covers the prices on both sides, which Sanitas plans include MRI, the waiting period and authorisation rules, and where to get scanned.
MRI scan cost in Spain: private prices against Sanitas
| How you pay | Cost per MRI | Notes |
|---|---|---|
| Sanitas Mas Salud (no copayments) | €0 | Covered in full once authorised |
| Sanitas Optima or Avanza | €12 copayment | Billed with the next premium; Avanza caps all copayments at €350 a year |
| Sanitas Mas Salud Plus | €0 to €10 | Counts as one service on the Plus scale: €0 for the first six services a year, then €4, €7 or €10 |
| Private, one body area, booked through Operarme (Madrid price) | €229 | €259 with contrast; results included; varies by city |
| Private, open MRI in Barcelona (Privaclinic list) | €85 to €159 | Knee from €85, depending on the study; small reservation fee |
| Private, executive clinic with an English report | from €375 | Single MRI at Executive Health, report in Spanish and English |
| Private whole-body MRI | $600 to $1,000 | Bookimed price guide for Spain, 2026 |
Private prices are the clinics' and booking services' own published prices for September 2026 and can change without notice; the Sanitas figures are the plan terms. A private scan usually needs a doctor's request, and check whether the report comes in English.
What moves the private price is the body area, whether contrast is used, whether the scanner is open or closed, and the city. The cheapest private scans are open-scanner studies of a knee or shoulder; contrast and sedation push the price up. On the policy the scan your doctor requests is covered once authorised, at the plan's copayment or none.
Will my Sanitas plan cover an MRI?
MRI and CT sit in the "complex diagnostic tests" group. Every plan in the table below includes them except Sanitas Accede, which is built around consultations and simple tests.
| Plan | MRI: what you pay | Wait for a new member |
|---|---|---|
| Mas Salud (and Mas Salud Family) | Covered, €0 | 6 months |
| Optima | Covered, €12 copayment | 6 months |
| Plus | Covered, one service on the copayment scale (€0 to €10) | 6 months |
| Sanitas Avanza | Covered, €12 copayment, €350 yearly cap | 6 months |
| Sanitas Unico (over 60s) | Covered with ultrasound and CT, €12 copayment per test | 6 months, waived on transfer from a full-cover plan |
| Sanitas Residents Visa | Covered, €0 | None |
| Sanitas Accede | Not covered, simple tests only | None |
If you have a reimbursement plan you can also use a non-network centre and claim a percentage back up to your plan's limits. Keep the invoice and the report and submit them through the app. See how copayments work for the full copayment ladders, or compare the Sanitas plans side by side.
The waiting period for an MRI
On Sanitas Mas Salud and most other plans a new member waits six months before a non-emergency MRI, CT or PET scan is covered. Three things override that:
- A life-threatening emergency is covered from day one.
- Sanitas Residents Visa has no waiting periods on anything.
- If you have held private health insurance in Spain for at least a year, Sanitas can waive the wait when you transfer. Transfers from the Bupa Global group onto certain plans are also possible, on the conditions set out on the switching page.
The full list of Sanitas waiting periods is in its own guide.
If you are already unwell, or already have a diagnosis, say so before you apply. That is handled as a pre-existing condition, not by the waiting period.
How to get an MRI with Sanitas
- Start with a specialist referral. Your Sanitas GP or specialist evaluates your symptoms and, if appropriate, requests an MRI.
- Authorisation. Complex diagnostic tests such as MRI require prior authorisation. You'll receive approval details in the app or by email/SMS, or you can obtain it from HealthPlan Spain.
- Book your appointment. Choose a centre from the Sanitas medical network. Many hospitals and diagnostic units let you book directly via the app or by phone.
- Attend the scan. Arrive early, remove metal objects and follow safety instructions. If contrast is needed, you may be asked about kidney function or allergies.
- Results and follow-up. A radiologist reports to your requesting doctor, who will discuss next steps and any treatment.
Tip: if you have implanted devices (a pacemaker, cochlear implant or metal fragments), inform the centre in advance. Some devices are MRI-conditional; the team will check safety before proceeding.
Where to get an MRI in the Sanitas network
Sanitas members choose from the radiology and diagnostic centres in the network. Our medical directory lists them by province: Madrid, Barcelona, Malaga, Cadiz, Baleares, or browse all diagnostic centres in the Sanitas network. What is an MRI scan?
An MRI uses a strong magnetic field and radio waves to create detailed images of organs, soft tissues, joints and blood vessels. Unlike X-rays or CT, MRI does not use ionising radiation. It is especially helpful for the brain and spine, muscles and ligaments, the abdomen and pelvis, and certain heart and vascular studies.
Common reasons a specialist sends you for one:
- Persistent back or neck pain, numbness or weakness suggesting a nerve or disc problem
- Unexplained headaches, fainting, seizures, or neurological symptoms
- Knee, shoulder, hip or ankle injuries where ligament or cartilage damage is suspected
- Abdominal or pelvic pain requiring a closer look at organs and soft tissues
- Monitoring known conditions (e.g., multiple sclerosis) or clarifying unclear ultrasound/CT results
MRI vs CT: what's the difference?
| Feature | MRI | CT |
|---|---|---|
| Imaging method | Magnetic fields and radio waves | X-rays (ionising radiation) |
| Best for | Soft tissues, brain/spine, joints, ligaments, pelvis | Bone, chest/abdomen emergencies, trauma, lung imaging |
| Scan time | 15-45 minutes (varies by body area) | About 15-30 minutes |
| Contrast | Gadolinium contrast sometimes used | Iodine-based contrast commonly used |
| With Sanitas | Same plans, same copayment where it applies, same 6-month wait | Same as MRI |
What an MRI appointment is like
You'll usually lie on a padded table that slides into a tunnel-like scanner. You must keep still while sequences are taken; you'll hear rhythmic tapping or humming. Ear protection is provided. If you are claustrophobic, tell the centre in advance and ask about an open scanner or mild sedation.
Gadolinium contrast is sometimes used to better visualise tissues and blood vessels. Adverse reactions are uncommon; the team screens for risk factors and monitors you during the scan. Hydration after contrast helps your body clear it; follow any written instructions you're given.
How quickly can I get an MRI?
Access times vary by location and clinical urgency. For urgent indications, your doctor can mark the request as priority. Non-urgent scans are generally scheduled soon after authorisation. Using the app and being flexible with nearby centres can speed things up.
Frequently asked questions
- How much does an MRI cost in Spain without insurance?
-
At published 2026 prices, a private MRI of one body area in Spain is listed from €85 to €159 on open scanners in Barcelona, and at €229 in Madrid through the Operarme booking service. An executive-health clinic that reports in English lists it from €375. Contrast adds to the price, and a whole-body MRI is quoted at $600 to $1,000. With Sanitas the same scan is €0 on Sanitas Mas Salud, a €12 copayment on Optima and Avanza, or one service on the Plus scale.
- Does Sanitas cover MRI scans?
-
Yes on every plan except Sanitas Accede, which covers simple diagnostic tests only. Sanitas Mas Salud, Optima, Plus, Family, Top Quantum, Avanza, Unico and Residents Visa all include MRI and CT when a Sanitas doctor requests the scan and it is authorised.
- Is there a waiting period for an MRI with Sanitas?
-
Six months on most plans for a new member, the same as CT and PET scans. Life-threatening emergencies are covered from day one. Sanitas Residents Visa has no waiting periods at all, and if you have held private cover in Spain for at least a year the six months can be waived when you transfer.
- How much does a CT scan cost in Spain?
-
With Sanitas a CT scan is treated exactly like an MRI. It is included on the same plans, €0 on Sanitas Mas Salud, a €12 copayment on Optima and Avanza, one service on the Plus scale, and a six-month wait for new members. If you are paying privately, the clinics that offer MRI usually offer CT as well, so ask for both prices.
- Do I always need a referral for an MRI?
-
Yes. A Sanitas GP or specialist requests the scan and provides the clinical indication needed for authorisation and scheduling. You cannot book an MRI directly on the policy.
- Will I pay anything on the day?
-
Not on Sanitas Mas Salud. On Optima and Avanza the €12 is billed with your next monthly premium rather than at the centre. You only pay a fee at the clinic if you have a reimbursement policy and use a non-network centre, in which case you claim it back afterwards.
- How much does an MRI with contrast cost in Spain?
-
Privately, contrast tends to make the scan dearer: Operarme lists one region with contrast at €259 against €229 without in Madrid. With Sanitas the scan is covered as your doctor requested it and Sanitas authorised it, at the plan's copayment or none.
- Are MRIs safe during pregnancy?
-
An MRI without contrast is used in pregnancy when it is essential, and preferably after the first trimester. Contrast-enhanced MRI is avoided unless strictly necessary. Your clinician will weigh risks and benefits.
- What if I am claustrophobic?
-
Tell the centre in advance. Ask about an open scanner or mild sedation. Arriving early and using breathing techniques can also help.
- Does Sanitas Accede cover an MRI?
-
No. Sanitas Accede covers unlimited consultations and simple diagnostic tests such as blood tests, X-rays and basic ultrasound. If you want MRI and CT on a copayment plan, Sanitas Avanza is the step up, with a €12 copayment per scan and a €350 yearly cap.
