Speech Therapy (Logopedia) in Spain: What Sanitas Covers, Session Limits and Who Prescribes It

Speech therapist working with a young child

Sanitas covers speech therapy (logopedia) on its full-cover plans for medical conditions: organic disorders of the larynx and vocal cords, and speech rehabilitation after a stroke. The limit is 80 sessions per person a year, prescribed by an ear, nose and throat specialist or a neurologist and authorised by Sanitas. Mas Salud Family covers 20 sessions a year and is the one plan that also includes articulation and voice-quality problems, a child's lisp among them. Language therapy for learning or developmental needs is excluded on every plan that covers speech therapy, and Sanitas Accede, Sanitas Avanza and Sanitas Unico do not cover the sessions.

This guide sets out what Sanitas covers, plan by plan: which conditions qualify, how many sessions, who has to prescribe and the waiting periods. It covers what children are and are not covered for, and what happens if the problem already exists when you apply. It then walks from a first appointment to a course of therapy, and ends with what a private logopeda costs if your case falls outside the cover.

Logopedia, logopeda, foniatria: what the words mean

Logopedia is the Spanish word for speech and language therapy, and a logopeda is a speech and language therapist, a university-trained health professional. Foniatria, phoniatrics in English, is the branch of medicine that deals with voice, speech and swallowing disorders. In Spain it is not a separate official specialty; it is practised by doctors, usually ear, nose and throat or rehabilitation specialists. That is why Sanitas calls the benefit "speech and phoniatric therapy" in English and "logopedia y foniatria" in Spanish. The doctor who usually diagnoses a voice or throat problem and prescribes the therapy is the ear, nose and throat specialist, the otorrino.

Therapists separate speech, producing sounds, from language, understanding and using words. Sanitas uses the same split: Mas Salud Family covers speech defects, and the under-14 allowance on the other plans covers language therapy. Two more words decide everything. Organic means there is a physical cause the specialist can see or diagnose: swelling, a nodule, a polyp, a tumour, a damaged nerve. A problem with no physical cause, a habit, a developmental delay, a learning need, is what Sanitas calls "without an organic disease", and it is excluded.

What Sanitas covers, plan by plan

Sanitas is precise about what speech therapy it pays for, and the list is narrower than most people expect. Every full-cover plan has the same 80-session rule. That is Sanitas Mas Salud, Mas Salud Optima, Mas Salud Plus, Top Quantum. So do Mas 90, Mas 90 Plus, Premium 500, Sanitas Professional, Professional Plus and Sanitas International Students. Mas Salud Family has its own rules, and two points that apply only to some plans are set out under the table.

Speech and phoniatric therapy cover by Sanitas plan, from the policy conditions
PlanCover
The full-cover plansUp to 80 sessions per person per year. Organic conditions of the larynx and vocal cords: swelling (oedema), benign nodules and polyps, cancer of the larynx, vocal cord paresis (weakness) or paralysis where a nerve or muscle is damaged, and congenital malformations. Speech therapy after an acute stroke. Prescribed by an ear, nose and throat specialist (larynx and vocal cords) or a neurologist (stroke), with prior authorisation from Sanitas after it reviews the medical report.
Mas Salud FamilyUp to 20 sessions per person per year. The same organic conditions, plus speech defects: difficulty producing the sounds needed to talk (articulation or phonological disorders, a lisp for example) and problems with voice quality, for adults as well as children. Other language problems, of comprehension or the correct use of words, are excluded. You need a written prescription from a Sanitas network doctor; send it to us and we will arrange any authorisation with Sanitas.
Sanitas Accede and Sanitas AvanzaNot covered. Both plans exclude speech and language rehabilitation as a complex therapeutic method, and Avanza also excludes any session with a speech therapy or phoniatrics specialist.
Sanitas UnicoThe consultation only. Treatment sessions are excluded as complex therapeutic methods.

Source: the Sanitas general conditions for each plan. Optima, Mas Salud Plus, Professional and Professional Plus carry the same 80-session rule. "Complex therapeutic method" is the policy's category for treatments that need specialist staff and equipment; chemotherapy and radiotherapy sit in it. Speech rehabilitation is a complex therapeutic method, which is why the entry-level plans exclude it and why the complex-therapy waiting period applies. Congenital malformations remain subject to the general rule on pre-existing conditions. Residents Visa: ask us before relying on it for speech therapy.

Two more rules apply on Mas Salud, Top Quantum, Mas 90, Mas 90 Plus, Premium 500 and the Students plan. Speech therapy after a stroke is covered only during the first six months after the acute event. And language therapy is also covered for children under 14, with a maximum of 20 sessions, alongside the organic list. The same two rules apply on Optima, Professional and Professional Plus.

Children: what is covered and what is not

This is where the old assumptions go wrong. Sanitas does not cover speech therapy because a child is struggling at school. Every plan that covers speech therapy excludes language education where there is no organic disease, and educational therapy. Every plan also excludes early care and cognitive stimulation. Top Quantum also excludes education therapy for under-16s. Treat a language delay, dyslexia, a stammer, selective mutism or feeding difficulties as not covered unless Sanitas confirms otherwise in writing.

What is covered for a child:

  • On Mas Salud Family: speech defects, up to 20 sessions a year. That means the child cannot produce certain sounds correctly (an articulation or phonological disorder, the classic lisp or sound substitution) or has a voice-quality problem. A lisp is not covered on plain Mas Salud, Optima or Plus, which cover organic larynx conditions and not speech defects. For an expat family whose child needs a logopeda, Family is the plan.
  • On Mas Salud, Top Quantum, Mas 90, Mas 90 Plus, Premium 500 and the Students plan: language therapy for a child under 14, up to 20 sessions. The 20 sessions are the child's allowance; ask us to confirm whether it renews each policy year. This means language work linked to a diagnosed medical condition, not a developmental delay. Sanitas decides at authorisation on the medical report. Do not buy a plan on the assumption that a developmental language delay will be accepted.
  • On every full-cover plan: the organic conditions, for example vocal cord nodules in a child who shouts a lot, or a congenital malformation of the larynx declared at application.

For everything else, families in Spain use three routes. Children under six can be assessed free of charge by the regional public early-intervention service (atencion temprana). The usual entry point is the public paediatrician, although the procedure varies by region and it assumes the child is registered with the public health system. State schools provide speech and language support through a specialist teacher, the maestro de audicion y lenguaje. And a private logopeda charges around €50 to €55 a session, which parents of a child with a mild articulation problem often find manageable for a short course. Our guide to children's cover with Sanitas covers the rest of paediatric care.

Adults: voice, throat and after a stroke

For adults the cover is built around two things. The first is an organic problem in the larynx or vocal cords. That means nodules and polyps from overusing the voice, which affect teachers, singers and anyone who talks for a living. It also means swelling, vocal cord paresis or paralysis after surgery or a nerve injury, and rehabilitation after treatment for cancer of the larynx. The ear, nose and throat specialist diagnoses it, usually by looking at the vocal cords with a laryngoscope, and prescribes the therapy. Being retired does not push you onto Unico: Mas Salud accepts new members up to 75 and keeps you insured after that.

The second is speech rehabilitation after an acute stroke, prescribed by a neurologist. On Mas Salud, Top Quantum, Mas 90, Mas 90 Plus, Premium 500 and the Students plan it is limited to the first six months after the stroke. The referral should therefore be made at the first neurology follow-up. Speech or language loss from any other neurological cause, a head injury, Parkinson's disease, multiple sclerosis or dementia, is not covered. The same rules that govern the rest of a stroke programme apply here: cover stops once functional recovery is reached or therapy becomes maintenance, and neuropsychological rehabilitation and cognitive stimulation are excluded. Swallowing therapy (for dysphagia) is not a listed benefit, so have Sanitas confirm in writing whether it forms part of the authorised course. A voice problem with no organic cause, for example hoarseness from strain with nothing found on the laryngoscope, is not covered on the full-cover plans. Mas Salud Family, however, covers "problems with voice quality" as a speech defect within its 20 sessions. Our guide to rehabilitation after a stroke explains how the wider recovery programme fits together.

If the problem already exists when you apply

Sanitas covers conditions that start after you join. Anything already diagnosed or being treated when you apply, a child's lisp, vocal cord nodules, a stroke, must be declared on the health questionnaire. Sanitas then accepts it, excludes it permanently from the policy, or declines the application. A stroke that has already happened will not be covered by a new policy, and a declared lisp or nodule may be excluded even on Mas Salud Family. Our guide to pre-existing conditions explains how Sanitas decides. The cover described here is for problems that arise once you are insured, which is why families take out the Family plan before a child needs it.

How to get speech therapy with Sanitas, step by step

  1. See the right specialist. For a voice or throat problem you can book an ear, nose and throat specialist directly, with no GP referral (the policy calls this free access). After a stroke the prescription must come from a neurologist in the Sanitas network. If the stroke was treated in a public hospital, book a network neurologist as soon as you are home and take the discharge report with you; the six-month clock is already running. For a child on Mas Salud Family, start with the paediatrician or the ear, nose and throat specialist so there is a diagnosis on file.
  2. Get the prescription and the report. The specialist issues a prescription (volante) for speech and phoniatric therapy and a medical report stating the diagnosis. Ask for both as documents and keep copies, whether or not they also appear in the app.
  3. Sanitas authorisation. On the full-cover plans Sanitas must authorise the course after reviewing the report. It works the same way as physiotherapy: send us the prescription and we arrange the authorisation with Sanitas for you.
  4. Book the logopeda. Use our Sanitas speech therapists by province or the app to choose a Sanitas network speech therapist and book the assessment. Take the authorisation, your card and the report to the first session. Use a Sanitas network therapist. Top Quantum does not reimburse speech therapy outside the network, and on Mas 90, Mas 90 Plus and Premium 500 ask us before booking a therapist outside the network.
  5. Count the sessions. The limit is 80 per person per policy year on the full-cover plans and 20 on Mas Salud Family; a new year resets it. Ask the therapist for a plan with a session count and review points.
  6. Copayments. On Optima, Plus, Mas 90 Plus and Professional Plus the plan's standard copayment applies to each session; ask us for the amount before you start. On the plans without copayments there is nothing to pay. Our guide to how copayments work explains the schedules.

Waiting periods for speech therapy

Mas Salud Family has a six-month waiting period for speech and phoniatric therapy, counted from the start date. On the other full-cover plans speech therapy falls under the waiting period for complex therapeutic methods: ten months on most plans and six on Top Quantum. Ask us for the date your speech therapy cover starts. Sanitas waives its waiting periods, except childbirth, when you transfer from another Spanish insurer you have held for at least a year, with proof of continuous cover. Residents Visa and the Students plan have no waiting periods at all. Our guide to waiting periods lists every period by plan.

Where the Sanitas speech therapists are

Sanitas has speech therapists in 49 provinces and autonomous cities, and our directory lists them by area with the clinic, town and number of therapists at each. In Madrid there are 19 clinics in 10 towns, in Barcelona 14 clinics in 11 towns, in Malaga 8 clinics in 6 towns from Marbella to Ronda, in Alicante 7 clinics and in Valencia 7. Some of Sanitas' own Milenium medical centres list logopedia and logofoniatria among their services. Speech therapy sessions are in person; a follow-up with the prescribing specialist can be a video consultation.

What a private speech therapist costs in Spain

If your case falls outside the cover, or you are not insured, an independent logopeda in Spain charges around €50 to €55 a session (2026 prices). One Barcelona practice charges €55 in person and €50 online, and the Fixando marketplace quotes an average of €50 a session for Conil de la Frontera in Cadiz. Multilingual therapy clinics charge more: Sinews in Madrid publishes €80 for a 50-minute speech therapy session and €150 for a joint session with a psychologist and a speech therapist. Against that, a covered course of 20 sessions on Mas Salud Family is worth about €1,000 at private rates. The full 80 sessions Mas Salud allows would be worth around €4,000, though a six-month post-stroke course is unlikely to use them all. Mas Salud costs from €89.53 a month for one adult aged 30 to 39; price Mas Salud and Mas Salud Family for your own ages.

Frequently asked questions

Does Sanitas cover speech therapy in Spain?

Yes, on its full-cover plans, for medical conditions. The full-cover plans cover up to 80 speech and phoniatric therapy sessions per person per year. They are Sanitas Mas Salud, Mas Salud Optima, Mas Salud Plus, Top Quantum, Mas 90, Mas 90 Plus, Premium 500, Sanitas Professional, Professional Plus and the Students plan. The cover is for organic conditions of the larynx and vocal cords and for speech rehabilitation after a stroke. The therapy must be prescribed by an ear, nose and throat specialist or a neurologist and authorised by Sanitas. Mas Salud Family covers 20 sessions a year and also includes articulation and voice-quality problems. Accede and Avanza do not cover speech therapy sessions, and Unico covers the consultation only.

Does Sanitas cover speech therapy for children?

Only in specific cases. Mas Salud Family covers speech defects up to 20 sessions a year. A speech defect means difficulty producing the sounds needed to talk (an articulation or phonological disorder, such as a lisp) or a problem with voice quality. Mas Salud, Top Quantum, Mas 90, Mas 90 Plus, Premium 500 and the Students plan also cover language therapy for children under 14, up to 20 sessions. Sanitas decides which conditions qualify when it reviews the medical report at authorisation. Language education where there is no organic disease, educational therapy and early-intervention programmes are excluded on every plan that covers speech therapy. So treat a language delay, dyslexia or a stammer as not covered unless Sanitas authorises it for a child under 14.

How many speech therapy sessions does Sanitas cover?

Up to 80 per person per year on the full-cover plans: Mas Salud, Optima, Plus, Top Quantum, Mas 90, Mas 90 Plus, Premium 500, Professional, Professional Plus and Students. Up to 20 per person per year on Mas Salud Family. The under-14 language therapy allowance on Mas Salud, Top Quantum, Mas 90, Mas 90 Plus, Premium 500 and the Students plan is 20 sessions; ask us to confirm whether it renews each year. Accede and Avanza cover none, and Unico covers the consultation only.

Do I need a referral for speech therapy with Sanitas?

You need a specialist's prescription, not a GP referral. On the full-cover plans you need a prescription from an ear, nose and throat specialist for voice and larynx conditions, or from a neurologist after a stroke. Sanitas then has to authorise the course after reviewing the medical report. You book those specialists directly; there is no GP gate. On Mas Salud Family you need a written prescription from a Sanitas network doctor; send it to us and we will arrange any authorisation with Sanitas. Start with the paediatrician or the ear, nose and throat specialist and we will confirm the route for you.

Is there a waiting period for speech therapy with Sanitas?

Mas Salud Family has a six-month waiting period for speech and phoniatric therapy. On the other full-cover plans speech therapy falls under the waiting period for complex therapeutic methods: ten months on most plans and six on Top Quantum. Ask us for the date your cover starts. Sanitas waives its waiting periods, except childbirth, if you have held health insurance with another Spanish insurer for at least a year. Residents Visa and the Students plan have none at all.

What does logopedia mean?

Logopedia is the Spanish word for speech and language therapy, and a logopeda is a speech and language therapist, a university-trained health professional. Foniatria (phoniatrics) is the medical discipline that deals with voice and speech disorders. In Spain it is not a separate official specialty; it is practised by doctors, usually ear, nose and throat specialists. That is why Sanitas calls the benefit speech and phoniatric therapy in English and logopedia y foniatria in Spanish. The ear, nose and throat specialist (otorrino) is the doctor who usually diagnoses a voice problem and prescribes the therapy.

Is a stammer, dyslexia or a language delay covered?

Not as named conditions. The Sanitas conditions cover organic conditions of the larynx and vocal cords, speech rehabilitation after a stroke and, on Mas Salud Family, articulation and voice-quality problems. They exclude language education and educational therapy where there is no organic disease, early-intervention programmes and cognitive stimulation, and stammering, dyslexia, language delay and swallowing difficulties are not listed benefits. The one opening is language therapy for children under 14, 20 sessions, on Mas Salud, Top Quantum, Mas 90, Mas 90 Plus, Premium 500 and the Students plan. Sanitas decides which conditions qualify when it authorises the course. Otherwise families in Spain use the public early-intervention services for under-sixes, school-based language support, or a private logopeda at around €50 to €55 a session.

How much does a private speech therapist cost in Spain?

Around €50 to €55 a session in 2026 at independent practices. Borja Tormos in Barcelona publishes €55 in person and €50 online, and the Fixando marketplace quotes an average of €50 a session for Conil de la Frontera in Cadiz. Multilingual clinics charge more: Sinews in Madrid publishes €80 for a 50-minute speech therapy session. With Sanitas the covered sessions cost nothing on the plans without copayments and a copayment per session on the plans with one. The amount depends on the plan; ask us before you start.

Does Sanitas cover speech therapy after a stroke?

Yes. Speech therapy after an acute stroke is covered on every full-cover plan, prescribed by a neurologist in the Sanitas network and authorised by Sanitas, within the 80-session annual limit. Mas Salud, Top Quantum, Mas 90, Mas 90 Plus, Premium 500 and the Students plan limit it to the first six months after the stroke. Start promptly. A stroke that happened before you applied is a pre-existing condition and will not be covered by a new policy. The same rules that govern the rest of a stroke programme apply: cover stops once functional recovery is reached or therapy becomes maintenance, and neuropsychological rehabilitation and cognitive stimulation are excluded.
