Family Health Insurance in Spain: What a Sanitas Family Plan Costs

Three generations of a family together outdoors in Spain

What does a Sanitas family plan cost in Spain?

Family health insurance in Spain with Sanitas means one policy for the whole household. In most of mainland Spain in 2026, two parents aged 40 with children of 6 and 10 pay €246.55 a month on Sanitas Mas Salud Family with no copayments. On Mas Salud Optima, with small copayments, they pay €172.43.

Everyone on the policy is priced by their own age band, the household discount comes off the total, and there is no age at which a child has to leave. Children see a paediatrician until they turn 15, and childbirth is covered after an eight-month wait for a pregnancy that begins after the policy starts. A newborn can join from birth with full rights when the conditions below are met and you apply within 30 days. One policy also satisfies the consulate for a family visa, with a certificate naming each person.

Who arranges it: HealthPlan Spain is an authorised Sanitas agent (agent code 22395) in Marbella that arranges Sanitas policies in English, at the same price as buying direct from Sanitas.

This guide explains how a Sanitas family policy works and who can be on it, and how a family of three, four or five is priced on each plan. It then covers what Sanitas Mas Salud Family (sold in Spanish as Sanitas Mas Salud Familias) changes against Sanitas Mas Salud, what children get, and the rules for pregnancy and newborns. It ends with the visa and residency rules for families and how the public system treats children. Prices are Sanitas' price list for new members from 1 April 2026 to 31 March 2027, paid monthly, in most of mainland Spain, Malaga included. Madrid, Barcelona, Valencia, Seville and several other provinces have their own tariffs, which the calculator applies from the province you choose. Madrid also asks for a postcode. A 0.15 per cent Consorcio de Compensacion de Seguros surcharge is added on the first receipt. A copayment, where the word appears, is a small fixed charge billed with a later premium, never paid at the clinic.

How a Sanitas family policy works

One policy, one region, one start date and one renewal date, with everyone on it insured on the same plan. The policyholder is an adult of 18 or over and is normally the first insured person. The others are insured members with their own card, their own health questionnaire and their own price. Sanitas puts no limit on how many people a family policy can carry. Children, partners, parents, grandparents, grandchildren, nieces and nephews can all join. A spouse or partner, married or not, joins as an insured member; our couples guide has the rules for two adults.

  • Children. A child under 14 is normally insured alongside the parent or guardian who has custody. Sanitas also issues a policy for a child of 4 or over on their own, with an adult of 18 or over as policyholder. A child of 0 to 3 always needs an insured adult on the same policy. There is no age at which a child must leave: the maximum age to join is 75, and there is no age limit to stay.
  • How each person is priced. By age band, meaning the age range that sets a person's price: 0 to 2, 3 to 15, 16 to 19, 20 to 24, then five-year bands. Babies are the dearest children, because the 0 to 2 band costs more than a 35-year-old on every Mas Salud plan. The 3 to 15 band is the cheapest, so the bill falls at the renewal after a child's third birthday. Each person's age on the start date sets their band for the year.
  • Underwriting. Each person answers the health questionnaire and is accepted, accepted with a condition excluded, charged more for a condition, or declined, on their own answers. Our guide to pre-existing conditions explains the outcomes. A newborn who meets the newborn rule below is accepted with full rights, without that step.
  • Adding someone later. A partner or child can be added to an existing policy through us with their identity documents and their own health questionnaire, on which they are accepted or declined. See adding a person to your policy. Ask us for the waiting dates for the person added before you rely on a date. Residence matters too: cover for a person ends if they move abroad or spend less than six months a year in Spain.

What a family pays with Sanitas in 2026

Two parents of 40 with children of 6 and 10 pay €246.55 a month on Mas Salud Family and €172.43 on Optima, with Mas Salud Plus between the two. The plans share one medical schedule: Mas Salud with or without copayments, and Family, which adds four things explained further down. Copayments per service are explained in our guide to how copayments work. The figures are monthly totals for most of mainland Spain, with the household discount already applied.

Three rules produce those numbers. A person insured alone on the Mas Salud plans pays an 8 per cent single-person loading, an extra charge for being the only person on the policy. It disappears as soon as two people share the policy. Mas Salud Family then takes 14 per cent off the whole household from three insured and 19 per cent from four or more; Mas Salud, Optima and Plus take 10 per cent off from four. Paying the year in one go takes a further 4 per cent off the discounted premium. Compare the plans on our calculator with your own dates of birth to see your household.

Two things follow from those rules. The crossover between Mas Salud and Family is the third person. Family costs a few euros more for one or two people and less for every household of three or more. Its band prices are a little higher, but its discounts start earlier. And the marginal cost of a child is smaller than the band price suggests. From a couple of 35, a first child of 4 adds about half as much a month on Family as on Mas Salud, because the third person triggers the household discount on everyone.

Mas Salud Family against Mas Salud: what changes

Both plans give the same cover for hospital treatment, surgery, tests, specialists, emergencies and travel assistance, and almost all the same exclusions. Dental 21 is included on both. Road and work accident cover is an optional add-on on both, free for under-18s, and included as standard only on Top Quantum and the Sanitas Professional plans. Family differs in four places:

  • Psychology. 20 sessions a year per person with no prescription and no authorisation, from day one, and 40 sessions a year for an eating disorder on a psychiatrist's prescription and report, authorised by Sanitas. Mas Salud, Optima and Plus give 15 sessions on a prescription from a psychiatrist, general practitioner (GP), oncologist or paediatrician, after a six-month wait. Our guide to psychology cover has the detail.
  • Speech therapy. Family covers 20 sessions a year for organic problems of the larynx and vocal cords and for speech impairments, after a six-month wait. That is where a child's lisp or trouble producing sounds sits; language delay and comprehension problems are excluded. Mas Salud covers 80 sessions a year for organic problems and 20 language-therapy sessions for under-14s. Both need a specialist's prescription and Sanitas' authorisation, and on Mas Salud the ten-month wait for complex treatments applies. Optima and Plus cover speech therapy on the same basis as Mas Salud. Our guide to speech therapy explains what each covers.
  • Family Assistance. Included on Family and an add-on at €2.20 a person on the others. It applies if an insured person is in hospital for more than 48 hours, is signed off sick and housebound for more than five days, or dies. Sanitas then arranges home help for up to 20 hours. For children under 16 it provides one of three things: a carer at home for up to 30 hours, a relative's fare to come to you, or taking the children to a relative. School runs twice a day for five days and up to three medicine deliveries a year are also covered.
  • Price. Family's band prices are a little higher at every age, and its household discounts start at three insured, against four on Mas Salud. Sanitas also sells a Family Plus version with the Plus copayments: the first six services free, then €4, €7 and €10, capped at €360 a person a year. It adds a €3 dental charge and a single €12 charge for high-frequency services such as a block of physiotherapy or antenatal classes. Family Plus is quoted on request.

Both plans have the same waiting periods for a new member: three months for minor operations, six for complex tests, eight for childbirth, and ten for hospital admissions and major surgery. Complex treatments such as chemotherapy and sterilisation also wait ten months. Psychology waits six months on Mas Salud and none on Family; speech therapy waits six months on Family. Everyday consultations, paediatrics, emergencies and simple tests have none. If you have held a full-cover policy with another Spanish insurer for at least a year, Sanitas removes the waiting periods, except childbirth. It needs the particular conditions and last receipt of that policy. Our guide to waiting periods lists every plan.

What children get on a Sanitas family policy

A child on any Mas Salud plan has the same network and the same hospital cover as the adults, plus the paediatric services set aside for them. Our children's health insurance guide covers the everyday illnesses, injuries, dental and eye care in detail; the policy facts are these:

  • A paediatrician until 15. Consultations at the surgery until the day the child turns 15, with no prescription needed. Home visits too, when the child is too ill to travel and Sanitas runs them in your area. You book the paediatrician directly; our guide to paediatrician cover explains how. The public system hands children to the family doctor at 14.
  • Preventive check-ups from birth to 11 under the paediatric prevention programme. The newborn's first 48 hours, including the medical check, vaccine administration and screening tests, are covered once the baby is added to the policy within 30 days of the birth.
  • Dental. Dental 21 comes free with the Mas Salud plans: more than 40 services including paediatric dentistry, the orthodontic X-ray study and the first fitting of braces, and up to 21 per cent off the rest. Orthodontic treatment itself is at the discounted price.
  • Emergencies and video. Sanitas emergency centres and a 24-hour emergency video service, with a paediatrician on it Monday to Friday from 8am to 10pm and the Sanitas 24 Hours doctors outside those hours. The Sanitas 24 Hours line is on most plans. A child under 18 uses video through a parent.
  • Not covered: the vaccines themselves. Every plan excludes the cost of vaccines. The consultation is covered and the vaccine itself is not; ask us before you buy a vaccine for a Sanitas nurse to give. For children registered with the public system the childhood calendar is given free at the health centre; a family whose residency rests on private insurance pays for the vaccine. Vaccination is not compulsory in Spain, but a nursery or school can ask to see an up-to-date vaccination record at enrolment.

Pregnancy and adding a newborn

Pregnancy is covered on Mas Salud, Family, Optima, Plus and Top Quantum for a pregnancy that begins after the policy starts. The birth or caesarean has an eight-month waiting period, the one wait Sanitas never waives on a transfer from another insurer. A premature birth before 37 weeks is exempt, for a pregnancy that began after the start date. Antenatal care, the hospital delivery with a midwife and a room with a bed for a relative are covered. So are up to ten pelvic-floor rehabilitation sessions in the year after the birth, if you are left with moderate to severe incontinence, with Sanitas' authorisation. In vitro fertilisation (IVF), home and water births and elective termination are not. If you are already pregnant when you join, the birth is not covered; the pregnancy is declared on the health questionnaire. Sanitas Residents Visa, sold only to applicants from outside the European Union (EU) applying from outside Spain, has no waiting period on childbirth for a pregnancy that begins after the policy starts. Our guide to pregnancy cover covers the early months.

Adding a newborn works like this. The baby can be included from the date of birth with all the policy's rights, and Sanitas covers the baby's own care, including the first 48 hours, once the baby is added. The baby qualifies when either parent has been insured on the policy for at least 240 days before the birth. The policyholder must apply within 30 calendar days of the birth, in the Mi Sanitas customer area, by phone or through us. Apply within the 30 days and the baby's cover runs from the day it is born. Outside those conditions, or after the 30 days, the baby is a new applicant and Sanitas can decline or apply the normal questionnaire and waits. Two practical consequences: have a parent on the policy at least 240 days before the birth, and diarise the 30 days.

Visas and residency for a family

Families from outside the EU applying for the non-lucrative visa must satisfy two rules. The first is the income test in Royal Decree 1155/2024: the main applicant shows means of 400 per cent of the IPREM, Spain's public income indicator, plus 100 per cent for each family member. With the 2026 IPREM of €600 a month, a couple with two children shows €4,200 a month. Family members are a spouse or a registered or stable partner, unmarried minor children, and adult children who cannot support themselves because of disability or ill health. Los Angeles asks for proof of financial dependence and also lists dependent parents. The second is the consulates' practice that each family member files their own application with their own insurance certificate. The London consulate wants cover for all the risks the Spanish public system insures; Los Angeles spells that out as no copayment, no deductible, no waiting period and no cover limit. One Sanitas policy covers the whole family and the certificate names each insured person, so nobody needs a separate policy.

The no-copayment plans the consulates accept include Mas Salud, Mas Salud Family, Top Quantum, Mas 90 and Premium 500. Their certificates show full cover with emergencies from day one, but the plans carry waiting periods. Where a consulate requires no waiting period, as Los Angeles does, Sanitas Residents Visa is the plan built for the application. It has no waiting periods and can be contracted up to five months before you arrive. The copayment plans are not accepted. Our visa health insurance page has the step-by-step. EU citizens registering as residents without working show sufficient means and health insurance covering all risks in Spain, for themselves and their family members, under Royal Decree 240/2007. Children under 21, or older and dependent, count as family members, and the registration is made within three months of arrival. One family policy does that job too, and a parent already registered as autonomo can use Sanitas Professional for the registration, though not for a visa. See residency registration.

The public system for children, and why families keep both

A child registered with the public system can choose a paediatrician until 7, a paediatrician or family doctor from 7 to 14, and the family doctor from 14. Vaccinations under the national calendar are free, and a dental programme runs to 14 (to 15 in Andalusia). Prescriptions carry a charge set by household income. What Sanitas adds is an appointment on a day of your choosing with any network paediatrician in Spain, a private room, and a specialist without a referral. Our guide to seeing a doctor with Sanitas compares the two routes. A family with public entitlement can keep both: vaccines and repeat prescriptions through the health centre, everything else through Sanitas.

Which plan for which family

  • Three or more of you, no visa in the picture: Sanitas Mas Salud Family. Cheaper than Mas Salud for every household of three or more, with 20 psychology sessions without a prescription, speech therapy for speech impairments and Family Assistance built in.
  • Two adults, or one parent and one child: Sanitas Mas Salud is a few euros cheaper than Family at two people; move to Family when a third person joins. Mas Salud Optima and Mas Salud Plus give the same cover for less if you accept copayments and do not need the policy for a visa or residency registration.
  • Applying for a visa from abroad: Sanitas Residents Visa for the application year, with no waiting periods, then review at renewal.
  • Parents over 60 or a student: Sanitas Unico and the International Students plan have no paediatrics. A child of 4 or over needs a Mas Salud policy of their own with a parent as policyholder, and a child under 4 needs a parent insured with them on a Mas Salud plan.

Ask us for the figures for your own household, or compare the plans on our calculator with everyone's date of birth; it applies the discounts for you.

Frequently asked questions

How much does family health insurance cost in Spain?

With Sanitas in 2026, two parents aged 40 with children of 6 and 10 in most of mainland Spain pay €246.55 a month on Mas Salud Family with no copayments. The same family pays €172.43 on Mas Salud Optima, with small copayments. Each person is priced by their own age band, then the household discount comes off the total, and paying annually takes 4 per cent more off.

Is there a family discount with Sanitas?

Yes, two kinds. A person insured alone pays an 8 per cent single-person loading on the Mas Salud plans, an extra charge for being the only person on the policy. It disappears as soon as two people share the policy. Then Mas Salud Family takes 14 per cent off the whole household from three insured and 19 per cent from four. Mas Salud, Optima and Plus take 10 per cent off from four. Optional add-ons are never discounted.

How old can children be on a Sanitas family policy?

There is no age at which a child has to leave. The maximum age to join is 75 and there is no age limit to stay. Each person is priced on their own age band: 0 to 2, 3 to 15, 16 to 19, 20 to 24 and five-year adult bands after that. Paediatric care runs until a child turns 15; from then on they see the same doctors as the adults.

Can a child have their own Sanitas policy?

A child under 14 is normally insured alongside the parent or guardian who has custody. Sanitas also issues a policy for a child of 4 or over on their own, with an adult of 18 or over as policyholder. A child of 0 to 3 always needs an insured adult on the same policy. Sanitas Unico and the International Students plan have no paediatrics, so a child needs a Mas Salud plan.

Can I add my partner to my Sanitas policy?

Yes: a spouse or partner, married or not, joins as an insured member on the same plan, region and renewal date, with their own health questionnaire and card. Our couples guide has the rules and what two adults pay. Ask us for the waiting dates for a person added mid-year before you rely on them.

How do I add a newborn baby to my Sanitas policy?

Apply within 30 calendar days of the birth and the baby joins from the date of birth with the policy's full rights. The baby qualifies when either parent has been insured on the policy for at least 240 days before the birth. Outside those conditions, or after the 30 days, the baby is a new applicant and Sanitas can decline or apply the normal rules.

Is pregnancy covered on a Sanitas family policy?

Yes on Mas Salud, Family, Optima, Plus and Top Quantum, for a pregnancy that begins after the policy starts. The birth has an eight-month waiting period that is never waived on a transfer; a premature birth before 37 weeks is exempt. Antenatal care, the delivery or caesarean in hospital and a room with a bed for a relative are covered. The newborn's first 48 hours are covered once the baby is added within 30 days. In vitro fertilisation, home births and elective termination are not. A pregnancy that already exists when you apply is declared on the health questionnaire and the birth is not covered.

Does Sanitas cover children's vaccinations?

No plan pays for the vaccine itself; vaccines sit on the medicines exclusion. The consultation is covered and the vaccine itself is not; ask us before you buy a vaccine for a Sanitas nurse to give. For children registered with the public system the childhood calendar is free at the health centre. Vaccination is not compulsory in Spain, but a nursery or school can ask to see an up-to-date vaccination record at enrolment.

What is the difference between Mas Salud and Mas Salud Family?

The medical cover is the same. Family adds 20 psychology sessions a year with no prescription and no waiting period, and speech therapy that includes articulation and voice problems (20 sessions, six-month wait). It also includes Family Assistance, with home help and childcare if a parent is in hospital. Its band prices are a little higher, but its household discounts start at three insured, so it is cheaper for every household of three or more.

Which is the best Sanitas plan for a family?

For three or more people with no visa in the picture, Mas Salud Family, which is cheaper than Mas Salud from the third person. It adds five more psychology sessions without a prescription, speech therapy that covers speech impairments, and Family Assistance. For two people, Mas Salud is a few euros cheaper. Optima or Plus give the same cover for less with copayments if you do not need the policy for a visa or residency registration. For a visa application from abroad, Sanitas Residents Visa for the first year, with no waiting periods.

Which Sanitas plan does a family need for a non-lucrative visa?

A plan with no copayments and no overall limit on hospital or medical cover in Spain: Mas Salud, Mas Salud Family, Top Quantum, Mas 90 or Premium 500. Those plans carry waiting periods. Where the consulate requires no waiting period, as Los Angeles does, Sanitas Residents Visa, which has none, is the plan built for the application. Each family member files their own application with their own certificate, but one Sanitas policy can cover the whole family and the certificate names each insured person.

How much income does a family need for the non-lucrative visa?

Under Royal Decree 1155/2024 the main applicant shows 400 per cent of the IPREM, Spain's public income indicator, plus 100 per cent for each family member. The IPREM for 2026 is €600 a month, so a couple with two children shows €4,200 a month, or €50,400 for the first year, plus health insurance for each of them.

Do expats get free healthcare in Spain?

Only with a right to the public system. The routes are paying Spanish social security, with your spouse and children as dependants, or a UK S1 as a state pensioner. The Convenio Especial buy-in after a year on the padron also qualifies. A family whose visa or residency rests on private insurance uses Sanitas for everything. Our public versus private healthcare guide explains the routes.

Does Sanitas sell a family plan with copayments?

Yes. Sanitas Mas Salud Family Plus uses the Plus copayment scale: the first six services a year free, then €4, €7 and €10, capped at €360 a person a year. There is also a €3 dental charge and a single €12 charge for high-frequency services such as a block of physiotherapy or antenatal classes.
